Job Description:
We are seeking a dedicated and compassionate Social Worker to join our team for a 13-week assignment in Syracuse, NY. The Social Worker will be responsible for providing comprehensive psycho-social assessments, interventions, management, and support to an at-risk patient population and their families. The role also includes assessing discharge planning and aftercare needs, addressing social determinants of health, and working collaboratively with a multidisciplinary team to ensure the best outcomes for patients.

Key Responsibilities:

• Conduct thorough psycho-social assessments for patients.

• Develop and implement intervention strategies and support services.

• Assist with discharge planning and assess post-discharge needs.

• Address social determinants of health, such as housing, food insecurity, and access to care.

• Collaborate with healthcare providers and the multidisciplinary team to ensure comprehensive care for patients.

• Participate in on-call/holiday coverage on a rotating basis as needed.

Experience Required:

• Minimum 3+ years of experience

Preferred Qualifications:

• A minimum of 3 years of experience working in a hospital or healthcare setting.
