Discharge Planner

Job Description:

We are seeking a compassionate and organized Discharge Planner to join our team at our facility in Dearborn Heights. The Discharge Planner will be responsible for coordinating and facilitating the safe and timely discharge of patients from the facility. This includes assessing patients' needs, developing discharge plans, coordinating with healthcare providers, and ensuring a smooth transition for patients to their next level of care.

Responsibilities:

• Assess patients' needs and develop individualized discharge plans
• Coordinate with healthcare providers to ensure continuity of care
• Arrange for necessary medical equipment, transportation, and home care services
• Provide education and support to patients and their families regarding post-discharge care
• Document all discharge planning activities in patients' medical records

Requirements:

• Valid RN or Social Work license
• Previous experience in discharge planning or case management
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of Medicare and Medicaid regulations
