Care Coordinator / Home Health – Full Time

Prestigious medical group specializing in advanced wound care in California has an immediate opening for a personable, self-motivated, and dependable full-time Home Health Care Coordinator to join our fast-paced and growing care team. This position requires a high level of attention to detail, strong office and phone etiquette, and excellent interpersonal and communication skills. The role involves daily use of electronic health records (EHR) to ensure provider notes are submitted accurately and promptly, regular communication with providers and referral agencies, as well as assisting with tasks and projects as needed. Medical terminology and direct patient interaction experience preferred. Candidates should be proficient in electronic health records and the Microsoft Office Suite (Excel, Word, Outlook, etc.).
