Synergy HomeCare of Tampa is seeking a Care Coordinator to support our caregivers and facilitate client care. Synergy Homecare of Tampa is growing quickly, and we understand the importance of providing exceptional care to our clients. We also recognize that caregivers are at the heart of our work. The Care Coordinator will be an experienced care provider who can offer guidance and support to caregivers. The Care Coordinator will also interact with clients and communicate with the office and with team leadership.

We are looking for an experienced care provider to perform supervisory duties and support our care team. A main goal of the Care Coordinator position is to establish a positive culture by enhancing employee satisfaction and morale in order to make Synergy HomeCare of Tampa a premier care provider. Strong candidates will have experience leading a team and overseeing hiring and retention. Because the Care Coordinator will also interact with clients, professional communication is an important element of the role. Additionally, a genuine interest in the health and satisfaction of our clients and caregivers is vital.

Qualifications:

  • Experience:
    • Exceptional caregiver with eligibility to provide care (Level-2 screening and CPR)
    • Supervisory or management experience in home care or other health related setting (preferred)
    • Scheduling (preferred)
  • Skills:
    • Leadership, customer satisfaction, organization, and time management skills
    • Exemplary communication skills, both verbal and written
    • Proficiency in major computer programs (Google Suite, Sheets/Excel, Power Point, etc)
    • Knowledge of scheduling software (AxisCare or similar) or CRM (preferred)
    • Knowledge of human resources management (preferred)
    • Knowledge of Relias or training platforms (preferred)

Experience as an exceptional care provider is required to guide and train caregivers and to provide direct client care as needed. The position includes providing leadership and management in areas related to caregiver recruitment, retention, and scheduling, as well as coordinating quality assurance visits. The Care Coordinator will be expected to understand the development of care plans and delivery of services, but will not be expected to complete assessments and intakes or to sell services.

Essential Duties and Responsibilities:

  • Recruiting, hiring, and onboarding new caregivers
    • Preparing and presenting orientation
    • Overseeing initial and ongoing training
    • Ongoing management, evaluation, and support of caregivers
    • Developing and deploying rewards, incentives, and retention plans
  • Maintaining employee files and records of certifications to ensure compliance
    • Recording and updating personnel files
    • Overseeing ongoing reporting of accountabilities
    • Compiling data from personnel, payroll, eRSP, or other sources to prepare reports
    • Preparing and filing incident reports
    • Auditing and maintaining files in accordance with federal, state, local, and company policies
  • Supporting clients and caregivers in the field across Hillsborough County
    • Performing warm handoffs to introduce clients to caregiver
    • Performing quality assurance calls and visits
    • Participating in the on-call rotation, call out coverage, and after-hours call coverage
