Job Title: Program Counselor

Job Duties: The Program Counselor is responsible for assisting in planning, organizing, and giving direct care to residents in the Activity Program and Social Treatment Program (STP) by current existing federal, state, and local standards, as well as our established policies and procedures. Responsible for ensuring that residents are given ample and regular opportunities to learn and practice skills for improving functioning, independence, awareness, self-care, and self-esteem. This role is also responsible for ensuring that residents' spiritual, emotional, recreational, and psychosocial needs are met/maintained individually.

Schedule:

  • Part-Time: Fri-Sat-Sun 10:30-7:00

Qualifications:

  • Minimum of a high school diploma/GED. Some college preferred.
  • Experience providing activities or another type of group treatment preferred.

About the campus: Crestwood Manor in Fremont is a skilled nursing facility serving individuals from a number of California communities. The facility provides long-term treatment services to people with a primary medical diagnosis and specialized behavioral management needs – for example, Alzheimer's or dementia.
