- Utica Park Clinic, founded in 1982, is a multi-specialty medical group with more than 300 employed physicians and advanced practice providers representing over 25 specialties across 80 plus clinics in Oklahoma.
- Communicate with Primary Care Provider and with multidisciplinary healthcare team regarding patient care needs.
- Serve as an advocate for patients/families in regard to decisions affecting the healthcare needs.
- Act as a resource to the multidisciplinary healthcare team for community resources
- Provide community resource support crisis intervention, disability and psycho education on chronic illness.
- Support the multidisciplinary approach for the management of suspected child/adult/elder abuse or neglect through early identification and appropriate referral for evaluation and management of these cases.
- Serve as an advocate for patients/families in regard to decisions affecting the plan of care
- Make appropriate referrals to home care, nursing home placement, hospice services, AL/IL and community agencies
- Document interventions and evaluations of care accurately and timely
- Remain knowledgeable about acts in accordance with the American with Disabilities Act and other state and federal laws protecting the rights of the client
- Remain knowledgeable about acts in accordance with legal principles of consent, healthcare proxies, and advanced medical directives
- Coordinate activities of the high risk case conference and develop plans of actions based on feedback.
Job Requirements:
- Post Graduate Degree (LCSW)
- Active member of the National Association of Social Worker
