Job Summary

A Home Health Intake Coordinatoris responsible for managing the process of admitting new patients into home health care services. This role involves gathering and verifying necessary patient information, coordinating with healthcare providers, and ensuring that all required documentation is completed accurately and timely. The Intake Coordinator serves as a liaison between the patient, family members, physicians, and the home health agency to facilitate smooth transitions and ensure that all care needs are met. Additionally, they assess patient eligibility for services, explain insurance coverage, and help schedule home visits with appropriate healthcare professionals.

Duties

  • Collecting patient medical histories and documentation
  • Verifying insurance information and eligibility
  • Communicating with physicians and healthcare providers
  • Coordinating home health services tailored to the patient’s needs
  • Ensuring compliance with healthcare regulations and protocols
  • Providing support to patients and their families throughout the intake process.

Requirements

  • Prior experience in medical office administration, patient intake, or case management is preferred.
  • Familiarity with medical terminology, home health care services, and industry protocols.
  • Understanding of patient care plans and healthcare documentation.
  • Excellent written and verbal communication skills for interacting with patients, families, physicians, and staff.
  • Ability to explain complex healthcare information in a clear, compassionate, and professional manner.
  • Strong ability to manage multiple tasks simultaneously, prioritize work, and maintain attention to detail.
  • Experience with managing patient records, appointments, and paperwork.
  • Basic knowledge of Microsoft Office (Word, Excel, Outlook).
  • Strong interpersonal skills and the ability to provide a positive experience for patients and their families.
  • Compassionate and empathetic attitude when handling sensitive situations.
  • Familiarity with insurance verification, patient eligibility checks, and the billing process.
  • Understanding of Medicare, Medicaid, and private insurance requirements for home health services.
  • Knowledge of healthcare regulations, such as HIPAA compliance, patient privacy, and patient rights.
  • Ability to identify and resolve issues efficiently, such as scheduling conflicts or insurance discrepancies.
