• 4:30 PM Guaranteed Hours: 40 hours per week Experience: Minimum 3+ years clinical experience in acute care or rehab setting License: LCSW
• Licensed Clinical Social Worker required; LICSW strongly preferred Certifications: BLS (AHA) Must-Have:
• COVID vaccination required (dose date on or after 10/3/23)
• Current knowledge of CMS, appeal rights, and DPH regulations
• Knowledge of Kepro QIO appeals
• Experience with utilization management and discharge planning
• Ability to perform comprehensive admission and discharge risk assessment
• Knowledge of Medicare, Medicaid, third-party commercial payers and reimbursement Job Description:
• Manage healthcare needs and facilitate achievement of quality and cost outcomes for patients across the continuum.
• Utilize a multidisciplinary team approach to negotiate, procure, coordinate, and monitor services and resources needed by patients.
• Review patients at admission and throughout the hospital stay for appropriate level of care and resource utilization.
• Ensure familiarity with regulatory and accreditation organizations such as Joint Commission, DPH, and HealthGrades.
• Demonstrate proven collaboration and negotiation skills with providers, patients, families, insurance organizations, and government agencies.
• Establish and maintain relationships with physicians and utilize local community resources.
• Maintain expertise in computer literacy and data management skills; ability to manage multiple open applications.
• Comprehend, research, analyze, interpret, and utilize data to prepare comprehensive summary reports as needed.
• Ensure confidentiality and excellent interpersonal and communication skills are maintained.
