• 4:30 PM Guaranteed Hours: 40 hours per week Experience: Minimum of 2 years in Acute Care Management License: Active License Required Certifications: BLS, LMSW or LCSW Must-Have: Experience with Epic Charting, Acute Care Management, and Level 1 trauma center Job Description:
• Fingerprint requirement can be completed remotely or in person 1 week prior to assignment
• Flu vaccine is required, with only medical exemptions allowed with documented evidence
• Must have previous experience in a large academic medical center or Level 1 trauma center
• Will cover all inpatient units throughout the hospital, focusing on medical discharge and social discharges
• Expected caseload is 15-20 patients per day
• The social work team consists of 15 full-time and 17 part-time social workers
• Will work directly with the Social Work Manager, with oversight from the Chief of Care Coordination
• Department does not have a Case Management Manager at this time
• No weekend or holiday requirements, but holiday rotation as needed
• Dress code allows for any color scrubs or business professional attire
• Department-specific orientation includes 2 weeks working alongside the Case Management team
• Responsible for consulting with RT and Pharmacy for discharge planning as needed
• Requires general understanding of discharge readiness
