Children's Medical Services is a Public Health program within the Family Health Bureau of the Department of Health.
Why does the job exist?
A primary function of this position is to provide direct medical social work - case management/care coordination services to children and youth with special health care needs- CYSHCN (and their families) in Rio Arriba and Los Alamos counties (primary counties) and other NE Region counties as needed/assigned. This position provides assistance with the coordination for pediatric specialty outreach clinics which provide access to care for the CYSHCN population in NM. This position provides a safety net and assurance services for children and youth with special healthcare needs. Services and concerns are addressed on both an individual/family level and on a systems level.
How does it get done?
Provide direct medical social work services to clientele in 6 NE NM counties (San Miguel, Mora, Colfax, Guadalupe, Harding and Union counties).and all other NE Region counties as needed and assigned. This position provides quality care coordination for Children and Youth with Special Health Needs who have complex medical, social and educational challenges provides coordination and access to care for pediatric specialty outreach clinics and a safety net and assurance services both an individual/family level and on a systems level for children and youth with special healthcare needs.
Coordinate support services, participate in staffing with schools, providers and other agencies identified as resources for the child/youth and families, while constantly advocating and acting as a liaison for the child in all general issues concerning the child's well-being.
Assist clientele in cultural brokerage and language interpretation with medical systems and providers. Ensure that detailed documentation is complete and current, and medical records are reviewed and shared with proper entities in order to provide the highest quality services for the client. Documentation is necessary to support Medicaid billing that this position brings in necessary to sustain program.

Minimum Qualification
Bachelor's degree in Social Work (in accordance with NMAC 61-31-9) and two (2) years of experience in a social work setting such as social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities, and other relevant experience, working with patients, clients, providers, and families. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience.
Employment Requirements
Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within six (6) months of date of hire as a condition of continued employment. Licensed by the New Mexico Board of Social Workers
Working Conditions
Work is performed in an office setting, client homes and community settings. Very minimal travel required. Work locations could be more than one county. Outreach to local/area in NE Region, Santa Fe, Rio Arriba, Los Alamos, San Miguel, Mora, Colfax, Taos, Union, Harding,and Guadalupe counties.
Clinic setting, constant public contact, computer usage, filing, involving stooping and reaching, may be required to lift up to 30 lbs.
