Qualifications and Skills:
- Hospice experience required.
- 5 years of experience with medical records required.
- Excellent communication and interpersonal skills.
Responsibilities/essential functions:
Must be able to perform the following essential job functions with or without reasonable accommodations.
- Answers incoming phone calls.
- Receives referral information and schedules evaluation appointments timely.
- Verifies Medicare or Medicaid benefits. Verifies private insurance and completes preauthorization requirements.
- Completes clerical duties, as necessary.
- Communicates with referral sources to ensure an open line of communication.
- Assembles patient medical records.
- Creates an electronic medical record and ensures medical records and admission documents are complete.
- Communicates with community liaisons to ensure proper follow up with referral sources.
- Maintains compliance with Medicare Conditions of Participation, State and Federal laws, accreditation standards, and hospice policy and procedures.
- Actively participates as a member of the team and demonstrates the organization mission and values in performance and behavior.
