JOB RESPONSIBILITIES
- Complete all required documentation in a timely manner.
- Attend meetings and trainings as required.
- Complete individualized service plans, progress notes and quarterly/discharge reports according to agency timeline and standards.
- Prevent further exacerbation of symptoms that would put the individual at risk of out of home placement or disruption in current living environment through implementation of coping skills/de-escalation techniques.
- Refer and link client to all necessary medical and/or behavioral health services.
- Coordinate with medical and/or behavioral health providers providing services to the individual throughout the delivery of the service as needed.
- Provided support and education on improving independent living skills such as hygiene, budgeting and securing and maintaining housing, behavioral management techniques, how to identify and utilize community resources, the importance of maintaining medication compliance for mental health and physical health stability.
- Respond to crisis situations with twenty-four hour a day availability.
- Maintain professional boundaries at all times.
- Meet with clients at scheduled times to address treatment goals and objectives.
- Make addendums to treatment plans as needed.
- Participate in monthly staff meetings, individual supervisions and treatment team meetings as scheduled.
Job Types: Part-time, Contract
Pay: $20.00 - $25.00 per hour
Schedule:

- Monday to Friday
- Weekends as needed
