JOB SUMMARY

Project Vida, Inc. is seeking an experienced Intensive Case Manager (ICM) to provide direct support to clients identified through referral sources who are living with or at-risk HIV, who are lost to care or at risk to being lost to care. The ICM will ma. This position will be required to perform multiple functions of the program including but not limited to outreach, linkage, testing and education. The goal of this position is to engage individuals who are living with HIV or at risk of contracting HIV into care and treatment, including newly diagnosed individuals and those who have fallen out of care.

KEY RESPONSIBILITIES

  • Develop comprehensive knowledge of Project Vida, Inc.’s history, mission and services.
  • Spend up to 60% of effort conducting field outreach to locate clients who are at risk or lost to care.
  • Assist in the development and implementation of an outreach plan to enroll persons living with or at risk of HIV into care.
  • Implement specified behavioral interventions as outlined by funding sources.
  • Asses client needs and personal support systems, develop an individualized needs assessment and service plan, coordinate services to implement the plan, monitor the efficacy of the plan and periodically re-evaluate the plan as related to clients’ on-going needs.
  • Empower client with support, education and community resources to access the services needed to fulfill the needs identified on client’s needs assessment.
  • Engage individuals in the community who are living with and at risk of HIV into care and treatment by performing outreach and linkage activities in high-risk HIV communities.
  • Maintain working relationships with partnering organizations.
  • Work cooperatively with supervisory staff and participate and provide all data and other information as requested by the program.
  • Maintain all program documentation and client records up to date.
  • Promote, articulate, and model the vision, mission and values of the grant program.
  • Must be able to work within the project team setup to provide client crisis intervention, staff coverage and be willing to share responsibilities as designated.
  • Collect all data required for program evaluation.
  • Other related duties as assigned.

PRIORITY QUALIFICATIONS

  • Education and/or Experience: Bachelor’s degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Previous Medical Case Management experience strongly preferred.
  • Current driver’s license, automobile insurance and accessibility to a vehicle is preferred.
  • Flexibility to work an alternative work schedule that includes nights and weekends.
  • Flexibility to attend meetings and trainings which may fall outside the regularly assigned work hours.
  • Excellent communication skills and ability for detail.
  • Display customer service excellence
  • Compassionate and able to relate to different clients with various needs.
  • Excellent organizational skills to manage several different cases at once.
  • Knowledge of HIV care and supportive services strongly preferred.
  • Bilingual English/Spanish strongly preferred.
  • Cultural/lifestyle sensitivity to African American, Latino, and LGBTQ+ populations required.
  • Ability to develop rapport and engender trust and maintain confidentiality.
  • Talent for ambiguity, rapid change and uncertainty; comfortable operating in an environment that can pivot as needed in response to internal and external priorities.
