We are currently seeking a conscientious, motivated individual to join our team as a Kentucky Veterans Center Social Worker Specialist. We are committed to serving Kentucky's Veterans by fostering independence, preserving dignity and providing compassionate care.

The candidate will assist in coordinating, evaluating and implementing the social service program at Thomson- Hood Veterans Center. Other responsibilities include but are not limited to the following:

• Provides professional social work and counseling services to the veterans and facility staff.
• Coordinates resident interaction with family and community support groups.
• Monitors the residents' social and emotional state, adjustment to the nursing home environment and treatment progress.
• Documents psychosocial histories, treatment plans, discharge plans and other pertinent data.
• Performs other duties as assigned.

The ideal candidate must have:

• Social Worker License Required
• Strong attention to detail
• Strong written and verbal communication skills
• Strong organizational skills
• The ability to multi-task Proficient computer skills
• Exceptional problem solving skills

Minimum Requirements EDUCATION: Graduate of a college or university with a master's degree in social work, sociology, psychology or related field.
EXPERIENCE, TRAINING, OR SKILLS: Three years of professional social work experience.
