Care Coordinator, Life Skills & Housing Supports
Full-time, Hourly
Must be available to work/manage flexible hours – mornings, evening, overnight
and weekend hours
REPORTS TO: Executive Director/Case Management Team Lead
JOB RESPONSIBILITIES: The Care Coordinator, Life Skills & Housing supports is responsible for coordinating the operations of a mid-barrier shelter/home. (1 home, up to 8 clients, and up to 5-7 team members). These responsibilities include understanding and assisting with the life skill goals of our clients, assist in the implementation and monitoring of a safe, healthy, outcome-focused shelter/housing environment, and leading a set of life skill shelter team members responsible for staffing the shelter 24/7/365.
SPECIFIC RESPONSIBILITIES:
Coordination of Operations (95%)
Commit to and role model C-U at Home’s core values and vision to serve those experiencing homeless.
Assist in implementing health and safety protocols for the organization.
Coordinate the shelter team to assist a client along his/her journey to meeting his/her life skill goals.
(Transportation, cooking, conflict resolution, resume building, to name a few)
Assist in recruiting, onboarding, training, and retaining strong life skills shelter team members.
Coordinate staffing schedule to ensure home(s) is staffed 24/7/365.
Serve on rotation for on-call emergency support for life skills team members.
Monitor, maintain and communicate for ordering an appropriate level of house supplies.
Monitor, maintain and communicate, if necessary, house maintenance needs.
Staff the residential space at least 32hr per week to ensure life skills goals are implemented in the
residential environment.
Maintain organization of all data, documents and necessary records and paperwork
Other (5%)
Participate in major events including planning and execution
Participate in community education on homelessness, when needed
Other duties as assigned
POSITION QUALIFICATIONS:
Minimum Qualifications
Associates Degree in social work, psychology, or related field experience working with the homeless
population or mental illness and/or substance abuse
2+ years of experience leading a team, scheduling, and coordinating activities
Ability to maintain confidentiality and confidential information
Good judgement and flexibility
Strong attention to detail and organizational skills
Strong verbal and written communication skills
Working knowledge of Microsoft and Google tools
Ability to relate effectively to diverse groups of people from all social and economic segments.
Preferred Qualifications
2+ years of experience working with the homeless population
3+ years leading a team
Working knowledge of data collection technology platforms

Pay: $25.00 per hour
