Position Summary: The Housing Case Manager at Sunrise Homeless Navigation Center plays a crucial role in our mission to provide comprehensive support and housing solutions for individuals experiencing homelessness. This position is responsible for providing case management services, collaborating with internal and external partners, and connecting referred clients to housing opportunities in the community. This position will work with Sunrise’s Housing team and help them accomplish the goal of connecting referred clients to housing in supportive ways that match Sunrise’s whole-person, client-centered approach.

Position Qualifications: A Bachelor's or Master's degree in Social Work, Public Administration, Nonprofit Management, or a related field.

  • Current or previous HMIS licensure preferred.
  • Bilingual (Spanish) is strongly preferred.
  • BSW or MSW preferred. Applicants with the ability to supervise Social Work interns will be given preference.
  • Demonstrated experience in case management and housing services, with a minimum of 2 years of relevant experience in PSH and/or RRH case management preferred.
  • Background in realty or landlord engagement preferred
  • Strong understanding of homeless services, housing best practices, harm reduction, the housing first model and the housing continuum.
  • Proficiency in data management, case noting and reporting.
  • Exceptional communication, interpersonal, and problem-solving skills.
  • Commitment to advancing housing stability and ending homelessness in the community.
  • Must have own transportation

Position Responsibilities:

A Housing Case Manager plays a crucial role in assisting individuals or families in securing stable housing and addressing any related challenges they may face. The Housing Case Manager’s responsibilities include:

  • Assessment: Conducting thorough assessments of clients' housing needs, preferences, and barriers to stable housing, such as financial constraints, legal issues, or medical concerns.
  • Goal Setting: Collaborating with clients to establish realistic and achievable housing goals and life skills, which may include finding permanent housing, maintaining current housing, or accessing supportive services.
  • Resource Referral: Connecting clients with community resources and support services, such as mental health services, substance abuse treatment, education opportunities, or job training programs.
  • Advocacy: Acting as an advocate for clients within the housing system, such as assisting with housing applications, negotiating with landlords, or advocating for fair housing practices.
  • Case Coordination: Coordinating services and referrals from multiple agencies or organizations to ensure that clients receive comprehensive support tailored to their individual needs.
  • Crisis Intervention: Providing immediate assistance and support to clients in crisis situations, such as homelessness, eviction, or domestic violence, and helping them access emergency housing and support services.
  • Documentation and Reporting: Maintaining accurate and up-to-date records of client interactions, service referrals, and progress towards housing goals.
  • Education and Empowerment: Providing information and education to clients about their rights and responsibilities as tenants, housing options, and community resources, empowering them to make informed decisions.
  • Follow-up and Support: Providing ongoing support and follow-up to ensure that clients remain stably housed and have access to the necessary resources and support services to maintain their housing stability.

Overall, Sunrise’s Housing Case Manager plays a crucial role in helping individuals and families navigate the complex process of securing and maintaining stable housing and addressing the various challenges they may encounter along the way.

  • The position requires working in an office, remote, and in clients’ homes.
  • The position is M-F, 9:00a-5:00p.
  • The position requires occasional flex work at the Sunrise Hub.

Job Type: Full-time

Pay: From $53,000.00 per year
